Cancellation Policy

Thank you for choosing Number One Lab. We are excited to serve you Our staff will make the specimen collection process as safe, quick, and comfortable as possible while providing excellent service and extremely fast result times

Our Collection Centers are the busiest from opening until 10:00am. If you are in a rush to get your test results we recommend that you schedule an appointment.

The Patient Should Always Bring The Following When Visiting A Lab:

• A completed Number One Lab requisition form from a Health Care Provider requesting Laboratory testing.
• A Valid insurance identification card. If in doubt please refer to our Insurance carrier acceptance link.
• A photo ID (i.e. A driver’s license or passport)
• A valid form of payment.

Note: Children must be accompanied by an adult and supervised at all times while in one of our Collection locations or facilities.
